伴抑郁的稳定期精神分裂症患者细胞因子
水平变化及其影响因素研究

【摘要】 背景 细胞因子水平与精神分裂症患者的症状存在相关性，同时多数精神分裂症患者在疾病进展后
期会出现抑郁症状，因此精神分裂症患者伴抑郁症状与细胞因子水平的关系值得进一步探讨。目的 探讨稳定期精神
分裂症伴抑郁症状患者细胞因子水平及其与临床症状的相关关系。方法 选择 2018 年 5—12 月安徽医科大学附属巢
湖医院、合肥市第四人民医院和马鞍山市第四人民医院精神科收治的稳定期精神分裂症患者。采用卡尔加里精神分裂
症抑郁量表（CDSS）评估患者的抑郁症状：以 CDSS 总分≥ 5 分定义为抑郁。使用流式细胞仪多重液相蛋白定量技术
检测患者血浆白介素（IL）-1β、IL-2、IL-6、IL-17A 水平。采用 Spearman 秩相关分析和多元线性回归分析探讨稳
定期精神分裂症患者细胞因子水平与抑郁的关系。结果 本研究共纳入 111 例稳定期精神分裂症患者，抑郁发生率为
28.83%（32/111）。Spearman 秩相关分析显示，CDSS 总分与 IL-1β、IL-2 及 IL-17A 呈正相关（r s 值分别为 0.507、
0.466、0.374，P<0.05）。多元线性回归分析结果显示，较高水平的 IL-17A 是稳定期精神分裂症患者伴发抑郁的保护
因素（B=-0.125，P<0.05），较长的总病程是稳定期精神分裂症患者伴发抑郁的危险因素（B=0.343，P<0.05）。结论
较高水平的 IL-17A 及较长的总病程与稳定期精神分裂症患者伴发抑郁存在相关关系。

【Abstract】 Background A large body of literature suggests that plasma cytokine levels are associated with symptoms
in patients with schizophrenia，but the relationship of plasma cytokines with depressive symptoms，which often occur in the late
stage of schizophrenia，still needs to be explored. Objective To explore the relationship between plasma cytokine levels and
depressive symptoms in schizophrenic patients. Methods Patients with stable schizophrenia were selected from Department of
Psychiatry of three hospitals（Chaohu Hospital Affiliated to Anhui Medical University，Hefei Fourth People's Hospital，and
Maanshan Fourth People's Hospital） from May to December 2018. The Calgary Depression Scale for Schizophrenia（CDSS）
