           新型冠状病毒肺炎对脑卒中患者病死率影响的 Meta 分析

               【摘要】 背景 新型冠状病毒肺炎(COVID-19)在全球范围蔓延,严重影响人类健康和生活。有研究报道
           COVID-19 可导致血栓性疾病,而脑卒中与血栓事件密切相关。目的 评估 COVID-19 对脑卒中患者病死率的影响,
           并对其可能机制进行探讨,从而为合并 COVID-19 的脑卒中患者的科学防治提供可靠的临床理论依据。方法 计算
           机检索 PubMed、EmBase、Web of Science、Cochrane Library、中国知网及万方数据知识服务平台等数据库,收集关
           于 COVID-19 对脑卒中患者病死率影响的队列研究或病例对照研究,检索时间为 2019 年 12 月至 2022 年 1 月。2 名
           研究人员独立进行文献筛选、资料提取,采用纽卡斯尔 - 渥太华量表(NOS)对文献质量进行评估。采用 Meta 分析
           评价 COVID-19 对脑卒中患者病死率的影响,采用漏斗图评价文献发表偏倚。结果 共纳入 18 篇文献,12 篇文献质
           量为高质量,6 篇文献质量为中等质量。Meta 分析结果显示,合并 COVID-19 的脑卒中患者病死率、凝血酶原时间
          (PT)、D- 二聚体水平、美国国立卫生研究院卒中量表(NIHSS)评分高于未合并 COVID-19 的脑卒中患者〔RR=4.16,
           95%CI(2.82,6.13),P<0.000 01;MD=0.78,95%CI(0.35,1.20),P=0.000 3;MD=1.34,95%CI(0.83,1.84),
           P<0.000 01;MD=6.66,95%CI(4.54,8.79),P<0.000 01〕,年龄低于未合并 COVID-19 的脑卒中患者〔MD=-2.04,
           95%CI(-3.48,-0.61),P=0.005〕。合并 COVID-19 的脑卒中患者和未合并 COVID-19 的脑卒中患者活化部分凝血
           活酶时间(APTT)比较,差异无统计学意义〔MD=2.51,95%CI(-2.69,7.71),P=0.34〕。对合并 COVID-19 脑卒
           中患者的病死率绘制漏斗图,结果显示两侧基本对称分布。结论 COVID-19 可增加脑卒中患者病死率,PT、D- 二聚
           体等凝血系统指标的改变可能在其中发挥重要的作用,其预后与年龄、入院时 NIHSS 评分等相关。

               【Abstract】 Background The COVID-19 pandemic seriously affects human health and life. COVID-19 has been
           reportedly associated with a high risk of thrombotic events,which are closely associated with stroke. Objective To assess the
           effect and possible mechanism of COVID-19 on stroke morbidity,providing a reliable theoretical basis for scientific prevention
           and treatment of COVID-19 in stroke. Methods We searched databases of Web of Science,PubMed,EmBase,Cochrane
           Library,CNKI and Wanfang Data for cohort studies and case-control studies related to COVID-19 and stroke published from
           December 2019 to January 2022. Two researchers conducted literature screening and data extraction separately. The Newcastle-
           Ottawa Scale was used to assess the quality of included studies. Meta-analysis was used to evaluate the impact of COVID-19 on
           stroke mortality. Funnel plot was used to evaluate the potential publication bias. Results A total of 18 studies were included,
           12 of them were of good quality,and other 6 were of fair quality. Meta-analysis showed that stroke patients with COVID-19 had
