针刺为主导治疗卒中后失语症疗效的系统评价再评价

【摘要】 背景 卒中是世界范围内导致患者死亡和残疾的主要原因之一，首次缺血性卒中后失语症发生率为
30%。目前指南推荐疗法通常难以获得满意疗效。诸多研究表明，针刺联合疗法可增加患者获益。目的 对针刺为主
导疗法治疗卒中后失语症的系统评价 /Meta 分析进行再评价。方法 计算机检索中国知网（CNKI）、万方数据知识
服务平台（Wanfang Data）、维普网（VIP）、中国生物医学文献服务系统（SinoMed）、Cochrane Library、PubMed 和
EMBase 数据库，搜集针刺治疗卒中后失语症疗效的系统评价 /Meta 分析，检索时间为建库至 2021 年 7 月。由 2 名研
究者分别进行文献筛选和资料提取。采用 AMSTAR 2 工具、PRISMA 声明、GRADE 分级评价纳入研究的方法学质量、
报告质量和结局指标的证据质量，对结果进行综合分析。结果 共纳入 12 篇系统评价 /Meta 分析，其中 1 篇方法学质
量为中质量，5 篇为低质量，6 篇为极低质量，主要由于研究者在系统评价 /Meta 分析中未对前期设计方案进行说明，
该项关键条目缺失导致多数研究为低质量或极低质量。12 篇文献 PRISMA 得分为 17~25 分，其中 20~25 分有 7 篇，报
告相对完整；17~19 分有 5 篇，报告存在一定缺陷。GRADE 分级结果显示，12 个为中级证据，24 个为低级证据，18
个为极低级证据。结论 针刺联合语言康复等疗法与单纯语言康复等疗法相比，在改善卒中后失语症患者语言障碍严
重程度、听语理解、复述及阅读能力方面，以及以针刺为主导疗法在提高卒中后失语症患者临床有效率方面，证据质
量均为中级，推荐临床使用。在其他方面，今后仍有必要进行更多高质量的原始研究，严格证据形成过程，为卒中后
失语症的针刺康复提供更有力的循证依据。

【Abstract】 Background Stroke is one of the leading causes of death and disability worldwide. The incidence of
aphasia resulting from first-ever ischemic stroke is about 30%. Current guideline-recommended therapy is often challenging to
achieve a satisfactory outcome，but numerous studies have shown that the treatment benefit could be increased if acupuncture
therapy is used in a combined approach. Objective To give an overview of systematic reviews/meta-analyses of acupuncture-
based treatment for post-stroke aphasia（PSA）. Methods Systematic reviews/meta-analyses of the efficacy of acupuncture-
based treatment in PSA were searched in databases of CNKI，Wanfang Data，VIP，SinoMed，Cochrane Library，PubMed
and EMBase from inception to July 2021. Literature enrollment and data extraction were performed by two researchers，
respectively. The AMSTAR 2，PRISMA statement，and GRADE classification were used to evaluate methodological quality，
the quality of reporting and the quality of evidence for outcome indicators，respectively. The results of overview were analysed
in a comprehensive manner. Results A total of 12 systematic reviews/meta-analyses were included. One was of moderate
