    Clinical Selection of Combined Lipid-regulating Drug Therapies

    Blood lipid regulating-therapy is an essential management in primary and secondary prevention of cardiovascular disease.Statins remain the cornerstone of modern lipid-regulating therapy.But statins hardly solve all types of dyslipidemia.In order to offer available combined drug therapies and related considerations for clinical practice,this paper summarized the combined therapies of lowing LDL-C and those of improving mixed dyslipidemia,answered the questions about whether application of these therapies could effectively reduce the cardiovascular risks and explored the application prospect of these therapies based on analyzing the population suitable for combined lipid-regulating drug therapies.

    For nearly three decades, lipid-lowering agents, especially statins, have been widely used for regulating blood lipids and preventing atherosclerosis for their safety and efficacy in lowering low-density lipoprotein cholesterol (LDL-C) and elevating high-density lipoprotein cholesterol (HDL-C), thwarting lipid infiltration and deposition in the vascular wall and stabilizing plaques. However, the adverse effects of this kind of drugs have been revealed by clinical trials and clinical applications, subsequently their clinical application has been limited. This paper elaborates on the adverse effects of lipid-lowering agents and the corresponding management measures, so as to provide guiding suggestions for clinicians to use such drugs scientifically and rationally.

    Cardiovascular and cerebrovascular events are the main causes of death in most diabetic patients.The risk of cardiovascular and cerebrovascular diseases in diabetic patients is 2-4 times higher than non-diabetic population.Diabetic patients are often associated with dyslipidemia,hypertension and other important risk factors for cardiovascular and cerebrovascular diseases.About more than half of the patients with diabetes have a disorder of lipid metabolism.And diabetes with blood lipid disorders is more likely to lead to the occurrence and development of major vascular complications.Therefore,in order to prevent the incidence of diabetic macrovascular disease,we must control the blood glucose at the same time carry out the active modulation of lipid therapy.In recent years,some authoritative guidelines in the field of cardiovascular disease were published in which some new proposals on serum lipid management were put forward.This paper analyzed some of the authoritative guidelines,and based on this,explored the reasonable strategies for lipid management in diabetic patients.

    Poor dietary can directly affect the incidence and prognosis of cardiovascular disease or by acting on cardiovascular disease risk factors.It is one of the academic hot spot to reduce the incidence and mortality of cardiovascular disease by adequate diet in recent years,but there are some controversial issues on the understanding of some subjects.In this paper,we analyzed the more controversial issues in detail,such as the relationship between cardiovascular diseases and some components of food including cholesterol,fatty acids and dietary supplements,and expected to draw more representative conclusions.We reviewed and analyzed the domestic and international dietary guidelines and relevant literatures in recent years,and found that it was important for preventing the occurrence of cardiovascular disease to restrict the intake of saturated fatty acids and trans fatty acids and increase dietary fiber intake.There was insufficient evidence for the various dietary supplements to prevent cardiovascular disease,and it should not be over-recommended.

    Background Biclustering is a more and more widely used data mining technique in recent years,and provides a more scientific analysis method for document management.Objective To analyze the research hotspots of general medicine,deepen digging the research themes and find the problems in general medical research by using biclustering method.Methods On March 1st,2016,journal literatures on general medicine were retrieved based on the data resources of China Academic Journals Full-text Database.Bibliographic Item Co-Occurrence Matrix Builder (BICOMB) 2.0 was used to filter prolific authors and high-frequency keywords,and gCLUTO 1.0 was used to conduct biclustering analysis.Results A total of 2 119 references were retrieved,4 075 authors and 9 155 keywords were obtained.Eventually 64 prolific authors and 59 high-frequency keywords were obtained.According to the actual clustering effects,the authors were clustered into five categories:cluster 0 included authors Jun Shao,Jing-quan Li and Kidd Michael,and the research topic was the development of the global family medicine;cluster 1 included 12 authors such as Feng Xiao,Wei-liang Lin,Li-jun Du,etc.and research topics included the cultivation of family doctors,family doctor system research and the experience exploration of family doctor service;cluster 2 included 33 authors such as Kan Zhang,Rui-jie Gong,Xue-ping Du,etc.and research topic was the cultivation of general medical talents,which was mainly divided into two aspects:general medicine education and on-the-job training of general practitioners;cluster 3 included 8 authors such as Man-xia Li,Wei-hong Ma,Qin Li,etc.and the research topic was on-the-job training and standardization training of general practitioners;cluster 4 included 8 authors such as Wan-xia Huang,Xin-wang Wang,Li-wei Zhang,etc.and research topics included the signing service of family doctors,and general practitioners’ teacher training.Conclusion The current domestic researches fields of general medicine can be divided into five hot research topics:the development of global family medicine,general medical education,training of general practitioners,family doctor service and system research of family doctors;and the research themes are increasingly detailed and deepened,but there are still many problems in the goal of academic education of general medicine,qualified teachers in general medicine,the basic and deep level research on family doctor systems.

    In recent years,more attention has been paid to diabetes self-management support(DSMS) globally.The role of internet-based DSMS,one of the main forms of DSMS,has significantly won increased recognition.Community-based DSMS and internet-based DSMS can be integrated to form a more efficient and feasible DSMS model.Internet-based DSMS can be a way to link hospital-based DSME and community-based DSMS and improve the hierarchical medical system as a continued service model that combines education,diagnosis,treatment,rehabilitation,nursing,and so on,all in one for diabetes in China.

    The TCM hospitals alliance is a new model of medical resources integration under the hierarchical medical system.In this article,based on the theory of "two points theory ",we analyzed the advantages and disadvantages in the development of the TCM hospitals alliance,and proposed stratagies from the aspect of the government,hospitals and society.In terms of the government,they should complete the legal status of the TCM hospitals alliance,promote innovation of medical insurance payment methods and "supply side of health care reform".In terms of TCM hospitals,they should define the position of all hospitals in the alliance,open two-way referral green channel and develop medical wisdom.In terms of society,media should publicize the correct way to medical treatment,increase the public awareness of the TCM hospitals alliance and encourage the TCM hospitals alliance to appropriately bring social capital in the development.

    Objective To analyze the difference of dialysis duration between death patients with dabetic nephropathy and death patients with non-diabetic nephropathy who underwent continuous ambulatory peritoneal dialysis (CAPD) during lifetime,and to analyze the correlated factors of dialysis duration among them.Methods A total of 43 death patients with end stage renal disease (ESRD) who underwent CAPD during lifetime in the First Affiliated Hospital of Guangxi Medical University from 2006 to 2014 were selected as study subjects.According to the primary disease,the patients were divided into diabetic nephropathy group (10 cases) and non-diabetic nephropathy group (33 cases).Clinical data including sex,age at the beginning of CAPD,dialysis duration,systolic blood pressure,diastolic blood pressure,pulse,height,degree of weight of lower limbs,degree of edema of lower limbs,levels of hemoglobin (Hb),C-reactive protein (CRP),serum albumin (Alb),potassium,calcium,phosphorus,blood urea nitrogen (BUN),creatinine (Cr),intact parathyroid hormone (iPTH),total cholesterol (TC),three acyl glycerin (TG),high density lipoprotein cholesterol (HDL-C),low density lipoprotein cholesterol (LDL-C),fasting blood glucose,ultrafiltration volume,the 4 h dialysate-to-plasma ratio (D/P) of creatinine,estimated glomerular filtration rate (eGFR),24 h urine volume,total liquid output (24 h ultrafiltration volume +24 h urine volume),peritoneal urea clearance (peritoneal KT/V),and total urea clearance (total KT/V) of patients at the beginning of CAPD were analyzed.Results There was no significant difference in sex,age,systolic blood pressure,diastolic blood pressure,pulse,height,degree of edemaof lower limbs,Hb,CRP,Alb,potassium,calcium,phosphorus,BUN,iPTH,TC,TG,HDL-C,LDL-C,ultrafiltration volume,D/P,eGFR,24 h urine volume,total content,peritoneal KT/V,and total KT/V between non-diabetic nephropathy group and diabetic nephropathy group (P>0.05).Dialysis duration 〔(39.1±24.4) months vs.(20.7±15.8) months〕and the level of Cr in non-diabetic nephropathy group were higher than those in diabetic nephropathy group (P<0.05).Weight and fasting blood glucose level in non-diabetic nephropathy group were lower than those in diabetic nephropathy group (P<0.05).Dialysis duration was not linear correlated with sex,age,systolic blood pressure,diastolic blood pressure,height,weight,degree of edema of lower limbs,Hb,CRP,Alb,potassium,calcium,phosphorus,BUN,Cr,iPTH,TC,TG,LDL-C,fasting blood glucose,ultrafiltration volume,and D/P (P>0.05).Dialysis duration was negatively correlated with pulse,and positively correlated with HDL-C,eGFR,24 h urine volume,total liquid output,peritoneal KT/V and total KT/V (P<0.05).Conclusion The dialysis duration of diabetic nephropathy patients receiving CAPD was shorter than that of non-diabetic nephropathy patients receiving CAPD.Pulse,HDL-C,eGFR,24 h urine volume,total liquid output,peritoneal Kt/V and total KT/V are correlated with dialysis duration.
    Clinical Characteristics of Peri-menopausal Female Coronary Heart Disease Patients with Different Blood Stasis Syndromes

    Backgroud In case of the risk factors of peri-menopausal women with coronary heart disease are basically same,there are significantly differences in the progress of coronary artery disease and incidence of cardiovascular events,the clinical features of peri-menopausal women with coronary heart disease need to be rethought.Objective To evaluate the clinical characteristics of peri-menopausal patients with coronary heart disease with different blood stasis syndromes.Methods 320 patients who were admitted to Beijing Anzhen Hospital,Capital Medical University during May 2014 to May 2016 and met the inclusion criteria,were selected as study subjects.According to patients’ age,combined chronic diseases and type of blood stasis syndrome(qi deficiency and blood stasis,phlegm and blood stasis,qi stagnation and blood stasis,other syndromes),they were divided into observation 1 group,observation 2 group,observation 3 group,and control group,with 80 cases in each group.General information 〔age,combined diseases(diabetes,hyperlipidemia and hypertension),coronary artery lesions〕,laboratory examination indicators 〔alanine aminotransferase(ALT),aspartate aminotransferase(AST),total protein(TP),albumin(Alb),three-glycerol(TG),total cholesterol(TC),high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),urea nitrogen(BUN),creatinine(Cr),uric acid(UA),fasting blood glucose(FBG),C reactive protein(CRP),homocysteine(HCY)〕,and auxiliary examination indicators 〔left atrial diameter,left ventricular end diastolic diameter,interventricular septum thickness,left ventricular posterior wall thickness,left ventricular ejection fraction(LVEF)〕 of patients were recorded.Results The proportion of one-vessel coronary artery lesions in observation 1 group and observation 2 group was significantly lower than that in control group,respectively,while the proportion of three-vessel coronary artery lesions,and the proportion of three-vessel coronary artery lesions+left main coronary artery lesions were higher than those in control group(P<0.05).The proportion of three-vessel coronary artery lesions in observation 3 group was lower than that in observation 1 group and observation 2 group,respectively,while the proportion of two-vessel coronary artery lesions in observation 3 group was higher than that in observation 2 group(P<0.05).The levels of TG,TC,HDL-C,and FBG in observation 1 group were significantly higher than those in control group(P<0.05).The levels of TG,TC,and FBG in observation 2 group were significantly higher than those in control group(P<0.05).The level of TC in observation 3 group was significantly lower than that in control group,while the level of FBG in observation 3 group was lower than that in observation 1 group and observation 2 group,respectively(P<0.05).There was no statistically significant difference in left atrial diameter,left ventricular end diastolic diameter,ventricular septal thickness,left ventricular posterior wall thickness,and LVEF among the four groups(P>0.05).Conclusion The severity of coronary artery disease among peri-menopausal women with coronary heart disease with qi deficiency and phlegm blood stasis syndromes is more severe than that among peri-menopausal women with coronary heart disease with other types,which may be related to more serious disorder of glycolipid metabolism.The fluctuation in blood pressure caused by emotional discomfort is the main cause of coronary heart disease attack among peri-menopausal women with coronary heart disease with qi stagnation and blood stasis syndrome.Targeted intervention and treatment based on clinical characteristics of blood stasis syndromes should be given in the early stage.
    Impact of Glomerular Filtration Rate on Prognosis in Patients with Acute Myocardial Infarction Undergoing Primary Percutaneous Coronary Intervention

    Objective To investigate the impact of glomerular filtration rate (GFR) on prognosis in patients with acute myocardial infarction (AMI) undergoing primary percutaneous coronary intervention (PCI). Methods A total of 118 AMI patients who were admitted to the Department of Cardiology of Liuzhou People's Hospital from June 2011 to July 2013 and met the inclusion criteria were enrolled in the study. According to the chronic kidney disease staging criteria issued by the National Kidney Foundation, the patients were divided into the normal estimated glomerular filtration rate (eGFR) group 〔eGFR≥90.0 ml·min⁻¹·(1.73 m²)⁻¹, n=56〕 and the low eGFR group 〔eGFR<90.0 ml·min⁻¹·(1.73 m²)⁻¹, n=62〕. General clinical data including gender, age, smoking history, body mass index, prevalence of hypertension, diabetes mellitus, dyslipidemia, serum creatinine level, Killip classification, serum cardiac troponin Ⅰ level, eGFR on admission, and left ventricular ejection fraction within 3 days after admission were recorded. Results of coronary angiography including infarct-related artery (IRA) and coronary artery lesion characteristics (number of diseased coronary arteries, presence of left main coronary artery disease) were recorded. All patients were followed up regularly after PCI, and the incidence of major adverse cardiovascular events (MACE), including target vessel revascularization, non-fatal myocardial infarction and cardiac death, was documented. Results There were no significant differences in gender, smoking rate, body mass index, prevalence of hypertension, diabetes mellitus, dyslipidemia, Killip classification, serum cardiac troponin Ⅰ level and left ventricular ejection fraction between the two groups (P>0.05). The age and serum creatinine level in the normal eGFR group were lower than those in the low eGFR group (P<0.05). No significant differences were found in IRA, number of diseased coronary arteries and prevalence of left main coronary artery disease between the two groups (P>0.05). There were no significant differences in the incidence of target vessel revascularization, non-fatal myocardial infarction and cardiac death between the two groups (P>0.05), while the total incidence of MACE in the low eGFR group was significantly higher than that in the normal eGFR group (P<0.05). The MACE-free survival rate in the low eGFR group was lower than that in the normal eGFR group (P<0.05). Cox proportional hazard regression model analysis showed that after adjusting for other confounding factors, age 〔RR=1.564, 95%CI (1.059,2.413)〕, diabetes mellitus 〔RR=3.916, 95%CI (1.874,7.215)〕 and decreased eGFR 〔RR=3.323, 95%CI (1.452,6.820)〕 were independent influencing factors for MACE (P<0.05). Conclusion Decreased eGFR in AMI patients with normal serum creatinine level is associated with adverse prognosis after primary PCI, and it is an independent predictor of MACE.
    Prevalence Rate and Risk Factors of Chronic Obstructive Pulmonary Disease in Xicheng District of Beijing

    Objective To investigate prevalence of chronic obstructive pulmonary disease(COPD) in Xicheng District,Beijing,and explore risk factors of its occurrence,so as to facilitate early intervention and standardized diagnosis and treatment of the disease in community health service institutions.Methods From July 2014 to July 2015,stratified cluster sampling method was used to extract 15 streets in Xicheng District,Beijing,and 1 125 residents were enrolled as research objects according to the constituent ratio of the population in each street in the total population of Xicheng District.The pulmonary function indices of residents were detected and the ratio of forced expiratory volume in 1 second(FEV1)/forced vital capacity(FVC) that less than 70% after inhaling bronchodilators was taken as the diagnostic criterion of COPD.The questionnaires were sent to residents for investigation.The contents of questionnaires included basic data 〔gender,age,body mass index(BMI),educational level,annual family income per capita〕,survival factors(smoking,using coal and biofuels,occupational dust exposure history,history of respiratory system disease,COPD family history,times of catching cold),and symptoms(cough,shortness of breath).Results A total of 1 125 questionnaires were issued,and 904 were recovered with an effective response rate of 80.4%.Seventy-five residents were diagnosed with COPD and the prevalence rate was 8.3%(75/904).There were significant differences in the prevalence of COPD in residents with different gender,age,educational degrees and annual family income per capita(P<0.05).The prevalence of COPD was significantly high in residents who smoked,had used coal and biofuels,had occupational dust exposure history,history of respiratory system disease and COPD family history(P<0.05);there was significant difference in the prevalence of COPD among residents with different times of catching cold(P<0.05).Smokers were divided into groups according to their SI(SI<200 pack-year,200 pack-year≤SI<400 pack-year and SI≥400 pack-year).The prevalence rate of COPD in smokers with SI<200 pack-year was lower than that in those with SI≥400 pack-year(P<0.05).The prevalence of COPD was significantly high in residents with symptoms of cough and shortness of breath(P<0.05).The results of multivariate Logistic regression analysis showed that age 〔OR=1.044,95%CI(1.014,1.074)〕,smoking 〔OR=5.485,95%CI(2.657,11.322)〕,using coal and biofuels 〔OR=1.731,95%CI(1.012,2.962)〕,having occupational dust exposure history 〔OR=2.407,95%CI(1.358,4.267)〕 and history of respiratory system disease〔OR=2.455,95%CI(1.409,4.279)〕 were the independent risk factors for residents developing COPD(P<0.05).Conclusion The prevalence rate of COPD in Xicheng District is 8.3%.Age,smoking,using coal and biofuels,having history of occupational dust exposure and of respiratory system disease are the independent risk factors for residents developing COPD.
    Aspirin Inhibition of PDGF-BB-induced Proliferation and Migration of Aortic Vascular Smooth Muscle Cells

    Objective To explore the effect and mechanism of aspirin on the proliferation and migration of aortic vascular smooth muscle cells induced by platelet-derived growth factor BB (PDGF-BB). Methods From August 2014 to September 2015, human aortic vascular smooth muscle cell line (T/GHA-VSMC) was selected and divided into four groups: control group (no reagent added), PDGF-BB group (10 ng/ml PDGF-BB added), aspirin group (5 mmol/L aspirin added), and PDGF-BB + aspirin group (10 ng/ml PDGF-BB and 5 mmol/L aspirin added). The absorbance (OD value) of T/GHA-VSMC proliferation at different culture time points (0, 24, 48, 72 h) in the four groups was detected by CCK-8 assay. The relative expression levels of p21waf1 and p27kip1 were detected by Western blotting. Cell cycle was detected by flow cytometry, and cell migration rate was detected by wound healing assay. The relative expression level of matrix metalloproteinase 1 (MMP-1) was detected by Western blotting. Results At 0 h of culture, there was no significant difference in the OD value of T/GHA-VSMC proliferation among the four groups (P>0.05). At 24, 48 and 72 h of culture, the OD value of T/GHA-VSMC proliferation in the PDGF-BB group was significantly higher than that in the control group, and the OD value in the aspirin group was significantly lower than that in the control group (P<0.05). At 48 and 72 h of culture, the OD value in the PDGF-BB + aspirin group was significantly lower than that in the control group (P<0.05). At 24, 48 and 72 h of culture, the OD values in the aspirin group and PDGF-BB + aspirin group were significantly lower than those in the PDGF-BB group (P<0.05), and the OD value in the PDGF-BB + aspirin group was significantly higher than that in the aspirin group (P<0.05). In the PDGF-BB group, the relative expression levels of p21waf1 and p27kip1 at 24 and 48 h were significantly higher than those at 0 h (P<0.05), and the relative expression levels at 72 h were significantly lower than those at 0, 24 and 48 h (P<0.05). At 24 h of culture, the relative expression levels of p21waf1 and p27kip1 in the PDGF-BB group were significantly lower than those in the control group, aspirin group and PDGF-BB + aspirin group (P<0.05). The proportion of cells in G0/G1 phase in the PDGF-BB group was significantly lower than that in the other three groups, while the proportion of cells in S phase was significantly higher than that in the other three groups (P<0.05). Wound healing assay showed that the cell migration rate in the PDGF-BB group was significantly lower than that in the control group, aspirin group and PDGF-BB + aspirin group at 24 h of culture (P<0.05). The relative expression level of MMP-1 in the PDGF-BB group was significantly higher than that in the other three groups (P<0.05), and the relative expression levels of MMP-1 in the aspirin group and PDGF-BB + aspirin group were significantly lower than that in the control group (P<0.05). Conclusion Aspirin inhibits PDGF-BB-induced proliferation and migration of aortic vascular smooth muscle cells by up-regulating the expression of p21waf1 and p27kip1 and down-regulating the expression of MMP-1.

    Background Research has confirmed the significant correlation between rs4129267 polymorphism on IL-6R gene and pediatric asthma suseptibility,but currently,the distribution of rs4129267 polymorphism among Han Chinese children and its correlation with serum IgE level have not been studied.Objective To investigate the correlation between rs4129267 polymorphism on IL-6R gene and pediatric asthma susceptibility and serum IgE level of Han Chinese children.Methods 410 asthma children from Department of Pediatrics of Guangdong Women and Children Hospital were selected as asthma group from 2013 to 2014.513 non asthma children in the same hospital were selected as control group during the same period.The DNA samples of two groups were extracted.Then the sites of rs4129267 and rs527236128 at IL-6R gene were genotyped by SNaPshot method.The total serum IgE level of two groups was determined by ELISA.Results The genetic distributions of rs4129267 and rs527236128 were accorded with Hardy-Weinberg equilibrium (χ2=3.264,2.135,P>0.05).The genotype frequency and allele frequency of rs4129267 were significantly different between the two groups (P<0.05).The genotype frequency and allele frequency of SNP rs527236128 were not significantly different between the two groups (P>0.05).The Logistic regression result showed that TT genotype was the risk factor to pediatric asthma (P<0.05).The total serum IgE level of control group was lower than that of asthma group (P<0.05).There was no significant difference in the total serum IgE level among different genotypes in two groups (P>0.05).The total serum IgE levels at all kinds of genotypes of control group were lower than those of asthma group (P<0.05).Conclusion rs4129267 polymorphisms on IL-6R gene is related to pediatric asthma suseptibility in Han Chinese children,but is not related to their serum IgE levels.

    Background The miR-200c can increase the sensitivity of tumor cells on paclitaxel and cisplatin,and the sensitivity of positive lung carcinoma cells in epidermal growth factor receptor(EGFR) on molecular targeted drug such as erlotinib hydrochloride tablets,gefitinib and afatinib.However,there are no related studies on whether it can increase the sensitivity of echinodermata microtubule-associated protein-like 4(EML4)-anaplastic lymphoma kinase(ALK) fusion gene positive lung adenocarcinoma cells on crizotinib,paclitaxel and cisplatin.Objective To observe whether miR-200c can increase the sensitivity of H2228 cells on crizotinib,paclitaxel and cisplatin,and to explore its mechanism.Methods From February 2014 to May 2015,H2228 cells were cultured and transfected.According to the different transfectants,the cells were divided into enhanced transfection group(transfected with miR-200c mimics),enhanced control group(transfected miR-200c mimics negative control),inhibitory transfection group(transfected with miR-200c inhibitor) and inhibitory control group(transfected with miR-200c inhibitor negative control).The real-time fluorescence quantification PCR method was used to detect the expression level of miR-200c gene in H2228 cells,MTT assay was performed to measure the proliferation level of H2228 cells under antineoplastic drugs〔ketazidine(50,100,200 mmol/L),paclitaxel(6.25,12.50,25.00 mmol/L) and cisplatin(25,50,100 nmol/L)〕 of different concentrations,Transwell assay was used to measure the migration rate of H2228 cells,Real-time PCR method was applied to detect the expression level of E-cadherin,N-cadherin,corrugated protein and CD24 mRNA in H2228 cells in transfection group and enhanced control group,Western blotting was used to measure the expression levels of E-cadherin,N-cadherin,corrugated protein and CD24 in H2228 cells in enhanced transfection group and enhanced control group.Results The gene expression level of miR-200c in H2228 cells in enhanced transfection group was significantly higher than that in the enhanced control group(P<0.05).The gene expression level of miR-200c in H2228 cells in inhibitory transfection group was significantly lower than that in inhibitory control group(P<0.05).The cell proliferation level of H2228 under the effect of antineoplastic drugs of different concentrations in enhanced transfection group was lower than that in enhanced control group(P<0.05).The proliferation level of H2228 cells under the effect of the effect of antineoplastic drugs of different concentrations in inhibitory transfection group was higher than that in inhibitory control group(P<0.05).The migration rate of H2228 cells in enhanced transfection group was greater than that in enhanced control group(P<0.05).The migration rate of H2228 cells in inhibitory transfection group was greater than that in the inhibitory control group(P<0.05).The expression level of E-cadherin and its mRNA in H2228 cells in enhanced transfection group was higher than that in enhancedcontrol group,and the expression level of N-cadherin,corrugated protein,CD24 and their mRNA was lower than that in enhanced control group(P<0.05).Conclusion Through the mesenchymal epithelial transiton in lung carcinoma cells,miR-200c can enhance the sensitivity of H2228 cells on crizotinib,paclitaxel and cisplatin.
    Effect of Rapamycin on the Expression of Vascular Endothelial Growth Factor in Airway Remodeling of Rats with Chronic Obstructive Pulmonary Disease

    Objective To explore the role of rapamycin on the expression of vascular endothelial growth factor (VEGF) in airway remodeling of rats with chronic obstructive pulmonary disease (COPD).Methods From June 2015 to March 2016,30 SPF grade male Wistar rats were randomly divided into three groups (normal group,model group and rapamycin group) by random number table method,and each group had 10 rats.The dual stimulation way of lipopolysaccharide (LPS) through the airway plus fumigation was conducted to rats in model group and rapamycin group to construct COPD rats’ model of airway remodeling,while no intervention was given to rats in normal group.Rats in rapamycin group received treatment of rapamycin,while rats in model group and normal group received the same amount of 0.9% solution of sodium chloride.After 14-day intervention,the serum,bronchial alveolar lavage fluid (BALF) and lung tissue specimens of rats in each group were collected;part of the right lung tissue was taken to perform HE staining and Masson staining and immunohistochemical staining were given to the remaining tissue,collagen thickness(WAc/Pbm) per unit length of small airway basement membrame,and the percentage of bronchioles VEGF positive expression area in the total area of the corresponding tracheal tube wall (Aim/WAt) were calculated respectively;the enzyme-linked immunosorbent assay (ELISA) was used to detect VEGF expression level in serum,BALF and lung tissue.Results The HE staining results showed that compared with model group,the pathologic change of bronchiole,muscular artery morphology and lung tissue of rats in rapamycin group had improved;Masson staining results showed that the WAc/Pbm of rats in model group and rapamycin group was greater than that in normal group,while WAc/Pbm of rats in rapamycin group was less than that in model group (P<0.05).Immunohistochemical staining results showed that Aim/WAt of rats in model group and rapamycin group was greater than that in normal group,while Aim/WAt of rats in rapamycin group was less than that in model group (P<0.05).ELISA testing results showed that the VEGF expression level of serum,BALF and lung tissue in rapamycin group and model group was higher than that in normal group,while the VEGF expression level of serum,BALF and lung tissue in rapamycin group was lower than that in model group (P<0.05).Conclusion Rapamycin can improve the airway remodeling of COPD rats,which may be related to the intervention treatment of rapamycin on decreasing of VEGF level in airway remodeling of COPD rats and thus inhibiting angiogenesis in rats.

    Objective To explore the effect and mechanism of autophagy on cardiac myocytes injury in uremic cardiomyopathy mice.Methods From August to December 2015,fifty male C57BL/6-PAX6 mice in a SPF grade were selected in our study.The mice were randomly divided into control group(6),sham operation group(8),test groups〔5/6 nephrectomy(NC) group (6),5/6NC+saline group(10),5/6NC+wortmannin group(10) and 5/6NC+Beclin-1 group(10)〕.The control group did not receive any intervention in the study.The sham operation group only received surgery and anesthesia procedures simultaneous with the test groups,but the kidneys were not removed.5/6 mice nephrectomy model were established in the test groups.Six weeks after model establishment,5/6 NC group did not receive intervention.5/6NC+saline group was given 0.9% sodium chloride solution 1 ml intraperitoneal injection,1 time every other day.5/6NC+wortmannin group was given wortmannin 1 mg/kg intraperitoneal injection,1 time/day.5/6NC+Beclin-1 group was given adenovirus carrying Beclin-1 1 ×109 pfu intramuscular injection,1 time/once.12 weeks after model establishment,mice heart function was measured by ultrasonic cardiograph,and renal function was measured by ELISA.In addition,we analyzed the expression of autophagy and its associated protein in mice myocardium using Western blotting and immunohistochemical staining after sacrificed.Results 12 weeks after model establishment,compared with control group and sham operation group,LVEDD significantly increased,LVFS significantly increased and serum creatinine and urea nitrogen level significantly increased in the 5/6NC group and 5/6NC+saline group(P<0.05).Compared with 5/6 NC+saline group,LVEDD significantly decreased,LVFS significantly increased,serum creatinine and urea nitrogen level significantly decreased in the 5/6NC+wartmannin group(P<0.05).Compared with control group and 5/6 NC+saline group,LVEDD significantly increased,LVFS significantly decreased,serum creatinine and urea nitrogen level significantly increased in the 5/6 NC+Beclin-1 group (P<0.05).Immunohistochemical staining showed that the expression of Atg5,Cathepsin-D and Rab 7 in the myocardial cells of 5/6NC group were significantly higher than that in control group(P< 0.05).There were significant differences in the rate of Atg5,Cathepsin-D and Rab7 positive cells between the control group and 5/6NC group(P<0.05).The expressions of autophagy-related protein LC3,Beclin-1 and AKT in the 5/6 NC+saline group and 5/6NC+Beclin-1 group were significantly higher than those in the control group (P<0.05).The expressions of LC3,Beclin-1 and AKT in 5/6 NC+wortmannin penicillin group were higher than those in control group,but lower than those in 5/6NC+saline group,5/6NC+Beclin-1 group (P<0.05).The expression of LC3,Beclin-1 and AKT in the 5/6 NC+Beclin-1 group were higher than those in 5/6 NC+saline group and 5/6 NC+wartmannin group(P<0.05).Conclusion Autophagy was closely related to cardiomyocytes injury in uremic mice.Enhancement or inhibition of autophagy could aggravate or relieve cardiac and renal function in uremic mice.Beclin-1 gene and PI3K/AKT signal proteins played an important role in regulation cardiomyocytes autophagy.

    Objective To explore the expression of calcium activated chloride channel (CaCCs) TMEM16A in rat atrial and ventricular tissues,so as to provide a new therapeutic target for clinical treatment of myocardial ischemia,arrhythmia and heart failure.Methods From October 2014 to September 2015,18 clean grade Sprague-Dawley rats were given 1 week of adaptive feeding,then,the heart was taken out after intraperitoneal injection of 10% chloral hydrate,and the atrial and ventricular tissues were isolated.The expression of TMEM16A and its mRNA in atrial and ventricular tissues were detected by Western blotting methods and real-time fluorescence quantitative reverse transcription PCR (RT-qPCR),respectively.Results The expression levels of TMEM16A in the rat atrial tissues were higher than those in the ventricular tissues〔(0.46±0.02) vs. (0.33±0.03),P<0.01〕.The expression levels of TMEM16A mRNA in the rat atrial tissues were higher than those in the ventricular tissues〔(3.55±0.17) vs. (1.00±0.03),P<0.01〕.Conclusion TMEM16A CaCCs was expressed in both atrial and ventricular tissues of rats,and the expression level in the atrial tissue was higher than that in the ventricular tissue.

    Background Cardiovascular diseases are a serious threat to public health, with approximately 16.7 million deaths worldwide each year. In China, coronary atherosclerotic heart disease (CAHD) is also the leading cardiovascular disease endangering public health, causing about 700,000 deaths annually. Among the treatment methods for CAHD, primary percutaneous coronary intervention (PCI) is the main treatment for ST-elevation myocardial infarction (STEMI). Objective To analyze the effect of optimized green channel on the delay time of primary PCI and prognosis in STEMI patients, so as to provide a theoretical basis for clinical emergency treatment of STEMI. Methods A total of 117 STEMI patients who underwent primary PCI through the optimized green channel in Zhengzhou People's Hospital from 2013 to 2014 and met the inclusion criteria were selected as the trial group. Retrospectively, 107 STEMI patients who underwent primary PCI through the pre-optimized green channel in Zhengzhou People's Hospital from 2011 to 2012 and met the inclusion criteria were selected as the control group. General data of patients, delay time 〔time from symptom onset to calling for help, 120 alarm response time, first-aid station assignment time, time from task acceptance to dispatch, time from dispatch to on-site arrival, time from on-site arrival to first medical contact, time from first medical contact to first electrocardiogram (ECG), time from first medical contact to oral administration of dual antiplatelet drugs (300 mg aspirin enteric-coated tablets and 600 mg clopidogrel), time from initial condition notification by emergency physicians to patient understanding, pre-hospital teleconsultation and catheterization laboratory pre-activation time, time from secondary notification and signature by consulting specialists, time from catheterization laboratory entry to balloon dilation, door-to-balloon (D2B) time〕, and prognostic factors 〔TIMI myocardial perfusion (TMP) grade, 30-day mortality, incidence of major adverse cardiovascular events after PCI (heart failure, arrhythmia, cardiogenic shock, reinfarction after reperfusion)〕 were recorded. Results Compared with the control group, the trial group had shorter time from symptom onset to calling for help, 120 alarm response time, first-aid station assignment time, time from first medical contact to first ECG, time from first medical contact to oral dual antiplatelet drugs, time from secondary notification and signature by consulting specialists, and D2B time, while longer time from initial condition notification by emergency physicians to patient understanding, pre-hospital teleconsultation and catheterization laboratory pre-activation time (P<0.05). There were no significant differences in time from task acceptance to dispatch, time from dispatch to on-site arrival, time from on-site arrival to first medical contact, and time from catheterization laboratory entry to balloon dilation between the two groups (P>0.05). The trial group had higher TMP grade, and lower 30-day mortality, incidence of heart failure, arrhythmia, cardiogenic shock, and reinfarction after reperfusion than the control group (P<0.05). Conclusion The optimized green channel can shorten the delay time of primary PCI and improve the prognosis of STEMI patients.

    Objective To explore the risk factors for the occurrence of capillary leak syndrome(CLS) in children with sepsis,in order to find grounds for early screening and timely treating the children with sepsis at higher risk.Methods A total of 321 children with sepsis admitted to the Department of Pediatrics of Hainan General Hospital from June 2007 to June 2015 were selected and divided into group without CLS (n=298) and group combined with CLS (n=23) in accordance with whether they had CLS.The period of observation for the children was between the admission time and discharge time.And during the period,we observed and compared the following between the two groups:clinical treatment results,age,gender,prevalence of malnutrition,white blood cell (WBC) count,serum procalcitonin (PCT) level,serum C-reactive protein(CRP)level,incidence of fasting blood glucose≥6.67 mmol/L,serum tumor necrosis factor-α(TNF-α) level,serum lactate level,occurrence of coagulation disorders,pediatric critical illness score(PCIS),prevalence of shock,incidence of multiple organ dysfunction syndrome(MODS) affecting 3 or more organs,usage rates of antibiotics treatment within 1 hour,and early goal-directed therapy within 6 hours.Multivariate Logistic regression analysis was used to analyze the influencing factors of CLS in children with sepsis.Results The clinical cure rate in the group without CLS was significantly higher than that in the group combined with CLS (χ2=41.309,P<0.001).The differences in terms of age,gender,prevalence of malnutrition,WBC count,usage rates of antibiotics treatment within 1 hour and early goal-directed therapy within 6 hours between the two groups were not statistically significant(P>0.05).Compared with the group without CLS,group combined with CLS had higher serum levels of PCT,CRP,TNF-α and lactate,higher prevalences of fasting blood glucose≥6.67 mmol/L,coagulation disorders,shock,and MODS affecting 3 or more organs,and lower PCIS(P<0.05).The results of multivariate Logistic regression analysis showed that the risk factors for the occurrence of CLS in children with sepsis were as follows:serum lactate level〔OR=2.609,95%CI(1.402,4.857),P<0.05〕,PCIS〔OR=0.634,95%CI(0.497,0.808),P<0.05〕,shock〔OR=0.124,95%CI(0.024,0.643),P<0.05〕 and MODS affecting 3 or more organs〔OR=0.120,95%CI(0.021,0.676),P<0.05〕.Conclusion Children with sepsis who have the risk factors as elevated serum lactate level,decreased PCIS,shock and MODS affecting 3 or more organs are apt to suffer from CLS.Intensively monitoring should be given to them as early as possible.

    Objective To observe the effect of different Chinese herbal compound on type Ⅰ,Ⅲ collagen in myocardium of rats with heart failure following myocardial infarction.Methods From April 2010 to January 2011,the rat model of heart failure following myocardial infarction were established by coronary artery ligation method and exhaustive swimming.The experimental rats were divided into model group,western medicine group,Reinforcing qi group,Activating blood group,Reinforcing qi and activating blood group,and control group.10 rats in each group.Real time fluorescence quantitative PCR and immunohistochemistry were used to detect the expression of type Ⅰ,Ⅲ collagen in the myocardium of rats with heart failure following myocardial infarction.Results Compared with control group,the expression of type Ⅰ,Ⅲ collagen in model group detected by PCR and immunohistochemistry significantly decreased (P<0.05).Compared with model group,the expression of type Ⅰ,Ⅲ collagen in each treatment groups detected by PCR and immunohistochemistry significantly increased (P<0.05).The expression of type Ⅰ,Ⅲ collagen in western medicine group and Reinforcing qi and Activating blood group detected by PCR and immunohistochemistry were significantly higher than Reinforcing qi group and Activating blood group (P<0.05).Conclusion Reinforcing qi and activating blood Chinese herbal compound can improve the expression of type Ⅰ,Ⅲ collagen in myocardium of rats with heart failure following myocardial infarction,thus achieve the role of treatment of heart failure.

    Objective To analyze the effects of three methods for regulating and invigorating the lung and kidney(lung invigorating and kidney tonifying,lung invigorating and spleen strengthening,and Qi supplementing and kidney nourishing) on the neutrophil-released reactive oxygen species(ROS) and neutrophil elastase(NE) stimulated by cigarette smoke extract(CSE) and Lipopolysaccharide(LSP).Methods From September 2014 to December 2015,96 conventional Japanese big ear rabbits were fed routinely for 1 week,then they were equally divided into 4 groups(normal group,lung invigorating and kidney tonifying group,lung invigorating and spleen strengthening group,and Qi supplementing and kidney nourishing group) according to random number table method,there were 24 rabbits in each group.Rabbits in lung invigorating and kidney tonifying group,lung invigorating and spleen strengthening group,and Qi supplementing and kidney nourishing group were intragastric administrated with lung invigorating and kidney tonifying,lung invigorating and spleen strengthening,and Qi supplementing and kidney nourishing formulas,respectively,rabbits in normal group were intragastric administrated with the same amount of normal saline.Blood samples are obtained from abdominal aorta after the 7th gavage,and the serum was segregated,then was saved in -70 ℃ for detection.Fresh blood was drawn from healthy male adult volunteers of Henan University of Chinese Medicine,and neutrophils were segregated.Under the stimulating of CSE,neutrophils were divided into normal group(20% normal group rabbit serum was added),CSE model group(20% normal group rabbit serum and 10% CSE were added),lung invigorating and kidney tonifying serum group,lung invigorating and spleen strengthening serum group,and Qi supplementing and kidney nourishing serum group(20% rabbit serum containing relevant formula and 10% CSE were added).Under the stimulating of LPS,neutrophils were divided into normal group(20% normal rabbit serum was added),LPS model group(20% normal rabbit serum and 2 μg/ml LPS were added),lung invigorating and kidney tonifying serum group,lung invigorating and spleen strengthening serum group,and Qi supplementing and kidney nourishing serum group(20% rabbit serum containing relevant formula and 2 μg/ml LPS were added).The activity of ROS and level of NE were detected by flow cytometry instrument and enzyme-linked immunosorbent assay(ELISA).Results The activity of ROS and level of NE in CSE and LPS group were significantly higher than those in normal group,respectively (P<0.05).The activity of ROS in lung invigorating and kidney tonifying serum group,lung invigorating serum group was significantly lower than that in CSE model group,respectively (P<0.05).While the level of NE significantly decreased in lung invigorating and kidney tonifying serum group,lung invigorating and spleen strengthening serum group,and Qi supplementing and kidney nourishing group (P<0.05).The activity of ROS in lung invigorating and kidney tonifying serum group,Qi supplementing and kidney nourishing serum group was significantly lower than that in LPS model group (P<0.05).While the level of NE significantly decreased in lung invigorating and spleen strengthening serum group and Qi supplementing and kidney nourishing serum group (P<0.05).Conclusion The serum with three methods of regulating and invigorating the lung and kidney can influence neutrophil-released ROS and NE stimulated by CSE and LPS.Under the stimulating of CSE,the serum containing lung invigorating and kidney tonifying drug is better than others in reducing the activity of ROS and level of NE.Under the stimulating of LPS,the serum containing Qi supplementing and kidney nourishing drug is the best in reducing the activity of ROS and level of NE.
    Acute Renal Injury Induced by Hemoglobinuria:Analysis of Two Cases and Literature Review

    Acute kidney injury (AKI) is a clinical syndrome that characterized by sudden decline in renal function.It is a critical illness in clinic.With poor prognosis and high mortality,its incidence rate was increasing year by year.Patients with acute renal injury caused by hemoglobinuria were not commonly seen in nephrological clinic.The paper introduces two AKI patients whose disease were caused by paroxysmal nocturnal hemoglobinuria and mechanical hemolysis,investigates this through combination of literature review and hopes to provide references for timely diagnosis and treatment of AKI patients who are caused by hemoglobinuria.

    Heparin-induced thrombocytopenia (HIT) refers to the thrombocytopenia,with or without thrombosis,caused by the use of heparin or low molecular heparin.Heparin or low molecular heparin is widely used in clinic,and the incidence rate of HIT is about 0.2% to 5.0% when they are continuously used for more than 4 days.The paper analysed the clinical data of a pulmonary embolism patient who appeared thrombocytopenia for many times in the treatment process when using low molecular heparin,reviewed related literatures,and analysed the pathogenesis,clinical features,diagnosis and prevention measures of HIT.

    In the past decade,the pain induced by piriform muscle degeneration becomes a problem hard to solve in the field of rehabilitation after spinal internal fixation in China.At present,there is no clear and effective treatment norm.This paper reported the individualized treatment for one case with pain caused by unilateral piriform muscle degeneration after three-dimensional correction of scoliosis with spinal fusion,bone grafting and internal fixation,with the mature biomechanical methods introduced from Hong Kong,TCM acupuncture precise-method of python wriggling for neuroregulation based on the improved image parameters,and oriented proprioceptive neuromuscular facilitation(PNF) for the correction of the affected limb from the perspective of compensation.It is the first successful report of the literature in China,which provides an exploration potential solution for the national shortage techniques needed by "rehabilitations back to the community".

    Cervical lymph node metastasis is the main transfer pathway of papillary thyroid carcinoma (PTC),the transfer order is usually from the central zone to lateral area.The diagnosis is difficult because of its occult onset,and the surgical scheme has not yet unified.Here we reviewed general rules,risk factors,diagnosis,treatment and prognosis of cervical lymph node metastasis in PTC,in order to help clinic screen high-risk patients with cervical lymph node metastasis of PTC and determine the reasonable treatment options.

    IgG4-related disease is a multi-system disease,which often manifests as elevated serum IgG4 concentrations,IgG4-positive plasma cell infiltration and fibrosis lesions.IgG4-related disease can affect many organs,such as pancreas,kidney,and salivary glands.When kidney is involved,we call it IgG4-related kidney disease,and its most common renal manifestation is IgG4-related tubulointerstitial nephritis.In addition,glomerulus involvement,most commonly membranous nephropathy(MN),could also be found.Meanwhile,idiopathic membranous nephropathy is associated with high IgG4.It is important to distinguish between the MN secondary to IgG4-related disease and idiopathic membranous nephropathy,which is a separate disorder.In this paper,through the discussion from pathogenesis,clinical manifestation,diagnosis and treatment,we compared the differences between IgG4-related kidney disease and idiopathic membranous nephropathy,in order to provide some theoretical foundations for clicinal diagnosis and treatment.
