Recent Progress on Transitional Care Assessment Tools for Older Chronic Disease Patients

Abstract

Abstract: Timely，appropriate and continuous care during hospital-to-home transitions may effectively reduce the readmission rate，emergency department visit rate and incidence of adverse events in older chronic disease patients. However，there is no unified evaluation tool for transitional care in China，which makes it impossible to objectively evaluate the effect of transitional care. We reviewed the latest progress on transitional care assessment tools for elderly chronic disease patients，summarized nine evaluation tools that evaluate the quality of transitional care from perspectives of patients and healthcare professionals，and the assessment contents and methods，reliability，validity and application of each tool were also expounded，with the aim of providing a recommendation for China's medical workers to select appropriate transitional care assessment tools.

摘要： 老年慢性病患者在医院-家庭过渡期间获得及时、恰当、连续的过渡期护理，可有效降低其再入院率、急诊就诊率及不良事件发生率。但目前我国对过渡期护理服务尚无统一的评价工具，无法对过渡期护理效果进行客观评估。基于此，本文对老年慢性病患者过渡期护理评估工具进行综述，归纳了从患者和医护人员两个角度对过渡期护理进行评估的9个评估工具，对各个评估工具的评估内容、评估方法、信效度及应用情况进行阐述与讨论，以期为医护人员选择合适的过渡期护理评估工具提供参考依据。
