摘要: 阻塞性睡眠呼吸暂停(OSA)是指睡眠过程中上气道完全或部分阻塞导致反复呼吸暂停和/或呼吸浅慢,进而导致微觉醒、间断低氧及白天过度嗜睡。OSA诱导的慢性间歇性低氧会引起交感神经兴奋、内皮功能障碍、全身炎症、氧化应激和代谢紊乱,均可诱发或加重心肌损伤。多项研究证实,OSA可以引起心肌损伤标志物升高、促进左心室重塑和功能下降,是心血管疾病发病和不良预后的独立预测因素。目前,OSA引起心肌损伤发生、发展的病理生理机制及其临床意义还有待深入研究。本文对OSA与心肌损伤关联的临床和基础研究进展进行综述。

Abstract: Obstructive sleep apnea(OSA) refers to the complete or partial obstruction of the upper airway during sleep,resulting in repeated apnea and/or hypopnea,in turn leading to microarousal,intermittent hypoxia and excessive daytime sleepiness.OSA-induced chronic intermittent hypoxia,can cause sympathetic nervous excitement,endothelial dysfunction,systemic inflammation,oxidative stress and metabolic disorders,can induce or aggravate myocardial injury.Many studies have confirmed that OSA can cause myocardial injury markers increasing,and promote left ventricular remodeling and decreased cardiac function,and is an independent predictor of cardiovascular morbidity and poor prognosis.At present,the pathophysiological mechanism and development of myocardial injury caused by OSA remains to be further studied.The objective of this review was to discuss the clinical and basic research related to OSA and myocardial injury.
